A major problem in American Medicine is the managed care concept of “medical necessity”…always used against the patient…s/he gets the insurance coverage in good faith, believes that s/he is protected, and finds that “medical necessity” is used to deny care. “Medical necessity” was never used to provide care, it will always be used against the patient.
